Jacob Perlow provides comprehensive hospice care for patients and their families. Our patients receive our reassuring care in their own homes, hospitals, hospice residences, or nursing homes. Jacob Perlow provides comprehensive hospice care for patients and their families. We respect the unique needs of each patient and their family and do our best to fulfill their individual wishes.

Our Interdisciplinary Care Teams are specially trained in comforting and alleviating the physical and emotional pain experienced by a family when a loved one is facing the end of life. Specially trained hospice physicians treat each patient for pain and other symptoms. Also, the patient’s own doctor may consult with our physicians on the patient’s disease and, when appropriate, other specialists may be brought in.

Nurses are the primary contact for patients. In addition to their medical duties, nurses order and coordinate all services, such as medication, home health aides and supplies.

A social worker is assigned to each patient and family for counseling and to help them cope; makes financial assessments and, when necessary, referrals for food stamps, housing, or other services and helps the family with funeral arrangements in advance of death.

Home health aides are available when necessary, to help with chores of daily living, such as meals, bathing, and wound dressing. Additional services such as transportation, nutrition, physical and speech therapies are provided as needed. And a member of the patient’s care team is on call for emergency and distress calls 24 hours a day, 7 days a week.

All necessary medical services are provided, including pharmacy and medication management, lab tests, radiology and medical supplies; and durable medical supplies such as a hospital bed or wheelchair.

Chaplains provide spiritual support for patients and families of any denomination. Bereavement counseling, support groups, outside referrals if necessary and memorial services are available for one year after the loved one’s death.

Most of our patients receive hospice care in their homes, at assisted or skilled nursing facilities or in inpatient units or hospitals. For those patients in need of inpatient care or who wish to reside in a hospice residence, Jacob Perlow has two highly respected facilities:

  • Jacob Perlow Hospice Inpatient Unit
    For patients in need of inpatient hospice care, our state-of-the-art hospice inpatient unit at Beth Israel Medical Center in Manhattan is the ideal choice. The Jacob Perlow Hospice Inpatient Unit is operated by MJHS Hospice and run in partnership with the highly respected Beth Israel Medical Center Pain Medicine and Palliative Care Department.
  • The Mollie and Jack Zicklin Jewish Hospice Residence
    Those who desire to live in a setting exclusively dedicated to hospice care will find our Zicklin Hospice Residence in Riverdale, a home for persons of all faiths, to be a wonderful choice. With 18 tasteful suites, Zicklin also offers skilled nursing services and enables residents to observe dietary and other religious practices in a homelike setting.

Jacob Perlow also provides hospice services at all four Continuum Health Partners hospitals (Beth Israel, St. Luke’s, Roosevelt and Kings Highway) and at nursing homes across the Greater New York area.
