MJHS provides comprehensive, compassionate hospice care for patients and their families in the comfort of their own homes, in hospitals, hospice residences or nursing homes. We respect the unique needs of each patient and their family and do our best to fulfill their individual wishes.
Our Interdisciplinary Care Teams are specially trained in comforting and alleviating the physical and emotional pain of a loved one who is facing the end-of-life. Specially trained hospice physicians treat each patient for pain and other symptoms. Also, the patient’s own doctor may consult with our physicians on the patient’s disease and, when appropriate, other specialists may be brought in.
RN Case Managers are the primary contact for patients. Each patient and family are assigned a hospice nurse to coordinate all services, such as medications, home health aides and supplies.
Social Workers provide ongoing counseling and support to help patients and families cope with the stress of serious illness. Assistance in planning is provided, which may include fi nancial assessment, referral to appropriate programs and services, and help with funeral arrangements in advance of the patient’s death.
Home Health Aides are available to help with activities of daily living, such as meals and bathing. Additional services such as transportation, nutrition, physical and speech therapies are provided, as needed. All necessary medical services are provided, including pharmacy and medication management, lab tests, radiology and medical supplies, and durable medical supplies such as a hospital bed or wheelchair.
On-Call services are provided; a member of the patient’s care team is on call for emergency and support 24 hours a day, 7 days a week.
Volunteer Services are available, when requested, to support the needs of the patient and family for companionship, errands, respite, etc. MJHS has a dynamic hospice volunteer program. Guided by compassion and caring, our volunteers help meet the physical, spiritual and psychological needs of our patients and their loved ones.
For more information on volunteer opportunities, contact our volunteer department at (718) 491-7139.
Chaplains provide spiritual support for patients and families of any denomination. Bereavement counseling, support groups, outside referrals if necessary, and memorial services are available for one year after the loved one’s death.
