Our Services

Whether you are with us to receive hospice or palliative care, our patients and families can expect an extraordinary level of attention and support.

Coordination of Care
Each patient and family is assigned an RN Case Manager, who will be their primary contact and will coordinate all services, such as medications, supplies, and if desired, visits with a social worker and home health aide. Social workers are available to provide ongoing counseling and support to help patients and families cope with the stress of a serious illness. They can also assist om applying for supportive services, such as Meals on Wheels and Medicaid. Our home health aides can help with activities of daily living, such as meals, bathing and dressing.

Care … However and Wherever It’s Needed
Because the needs of every patient are unique, we offer a wide variety of care options for adults and children. Our team of doctors and specialists provide a complete spectrum of services from care in your home, with regularly scheduled visits, to continuous care, where a nurse and/or a home health aide will remain in your home as an added level of support until symptoms are under control. The patient may also choose to be admitted to one of our hospice inpatient care units or residences, where nurses are available around the clock to administer medications, treatments and provide emotional support to make the patient more comfortable. We also offer Respite Care, which provides a temporary break for caregivers.

Round-the-Clock On-Call Services
You and your family can count on our round-the-clock on- call services. We know the needs of patients and their families change by the day or even the hour. Our professional staff is on call 24 hours a day, seven days a week. For a list of our on-call numbers click here.

Volunteer Support
MJHS has a dynamic volunteer program guided by compassionate and caring members who assist in enhancing the quality of life for patients and their loved ones. Volunteers are available for companionship, errands, respite care and bereavement support for families as well as visiting patients in homes, hospitals and nursing homes.
